Successful Treatment for Chronic Refractory Low Back Pain with Transforaminal Endoscopic Lumbar Annuloplasty: A Case Study

Abstract

|
The cause of chronic low back pain varies and it is presumed that in many cases, the pain originates from the intervertebral disc. Transforaminal endoscopic lumbar annuloplasty (TELA) is a recently developed procedure in which annuloplasty is performed with a laser, using an endoscope inserted into the epidural space through the intervertebral foramen. We performed TELA on a 21-year-old male patient who had suffered from low back pain and radiating pain of the left lower leg for several years. After the procedure, the patient¡¯s pain was much improved without any acute neurological complications. The annuloplasty during TELA may decrease the intradiscal pressure and block the nociceptive pain signal transmission, which is thought to be related to the discogenic low back pain. In conclusion, TELA may be an effective and alternative method for the treatment of carefully selected patients with internal disc disruption or small herniated nucleus pulposus.
